We're actively recruiting reliable caregivers who can travel to the Hammond area for assigned patient-care shifts.
Available shifts, days, and weekly hours depend on the patient's care needs and scheduling.

HHA / CNA Responsibilities
As a Home Health Aide or CNA, you'll provide personal care and support under the direction and supervision of Elite's nursing team and according to the patient's individualized Plan of Care.
Responsibilities may include:
- Assisting with bathing, dressing, grooming, toileting, and other activities of daily living (ADLs)
- Supporting safe mobility, transfers, positioning, and ambulation as directed
- Helping the patient maintain comfort, dignity, and independence
- Assisting with household tasks essential to the patient's care at home
- Observing and reporting changes in patient condition to the supervising nurse
- Assisting with self-administered medications only as permitted by applicable requirements and Elite policy
- Completing accurate and timely daily care documentation
- Following infection-control, safety, and patient-confidentiality procedures
- Communicating effectively with the patient, family, and clinical team
Qualifications
- Current HHA or CNA qualification, or willingness to complete applicable Home Health Aide training
- At least 18 years of age
- Compassionate, dependable, and committed to quality patient care
- Ability to read, write, communicate effectively, and follow the patient's Plan of Care
- Reliable transportation to the assigned patient location in Hammond
- Ability to safely perform assigned personal-care and physical duties
- Willingness to complete applicable training, competency evaluation, screening, and employment requirements
Previous home health, CNA, caregiving, personal care, or private duty experience is valued.
