Elite Home Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for a current home health patient need in Terre Haute, Indiana.
As a Home Health Field Nurse, you'll provide one-on-one skilled nursing care in the patient's home, allowing you to focus on the individual needs of your patient while building meaningful relationships with the patient and family you serve.

Current RN/LPN Patient Need
We currently have a skilled nursing need for an RN or LPN in Terre Haute, Indiana, based on patient care requirements and the assigned Plan of Care.
Additional patient hours may be available based on staffing, patient needs, qualifications, and scheduling. Specific shift details will be discussed during the hiring process.

Home Health Field Nurse Responsibilities
As a Home Health RN or LPN, you will provide skilled nursing services within your licensed scope of practice, in accordance with the patient's Plan of Care, physician orders, and applicable supervision requirements.
Responsibilities include:
- Assessing or monitoring patient conditions and documenting observations within the nurse's scope of practice
- Monitoring vital signs, medications, and overall patient health
- Administering prescribed medications and treatments as ordered and authorized
- Providing skilled nursing care according to the patient's individualized Plan of Care
- Educating patients and families on home healthcare procedures, medications, and care routines
- Coordinating and communicating with physicians, supervising RNs, and other healthcare professionals as appropriate
- Identifying and promptly reporting changes in patient condition
- Completing accurate, thorough, and timely clinical documentation
- Promoting patient comfort, safety, and overall well-being
- Following infection-control, patient-privacy, safety, and Elite Home Healthcare policies and procedures
Qualifications
Candidates must:
- Hold a current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license authorizing practice in Indiana
- Demonstrate strong clinical nursing skills appropriate to their licensure
- Possess excellent written and verbal communication skills
- Be dependable, compassionate, organized, and detail-oriented
- Demonstrate effective time-management and critical-thinking skills
- Be comfortable providing individualized nursing care in a home setting
- Be able to follow physician orders, individualized Plans of Care, and applicable clinical supervision requirements
- Work effectively with patients, families, and the interdisciplinary healthcare team
- Complete applicable Elite Home Healthcare employment, credentialing, screening, and compliance requirements
Preferred Experience:
Previous experience in home health nursing, private duty nursing, pediatric or adult skilled nursing, community-based nursing, or related clinical settings is valued.
